The original mid-blade bosses, and the crown (which he seems to have modified somehow). The extra bosses are for the pannier rails, and are spaced for bottle cages also

I realised my question is not clear, I ment what's the hardware on the rack? In the small perpendicual tubing on the struts and on the piece of rack that's mounted to the fork crown?

Not sure, can you circle which bits you mean?
Edit - sorry, I see now. On the struts, just cap-head bolts. Not sure on the crown! Presumably another bolt threaded in from the back
