Planning to spend a bit of my bonus money on a fit. I'm not a hardcore long distance rider but I'm fed up of having pains, plus having one leg longer than the other must be affecting me.
My right knee is already fucked whether I'm on or off the bike but this past week my left knee has been hurting whilst pedaling.

We'll see, I'm all out for spending this month (already haha).
There is something incredibly satisfying and also reassuring about riding your bike around and knowing in your mind "my bike is set up as perfectly to my body as it can be".
You won't ever have thoughts like "hmm, maybe if I twist my cleat a bit and move it back it will fix that niggling pain I get in the side of my knee", and then do it, and then the next time you ride you get a new pain and think "hhmmm, maybe I went too far, or maybe i went the wrong way. I'm going to change it when I get home". Then you go home and change it, then you get another pain and you think "hmm, maybe I had it right the first time and I've just made it worse, I'm going to move it back again".
The cycle never ends Fuck all that. Get the fit done. Take shit loads of photos of your cleats and bike afterwards and know you have something proper. And when you get pain, which you will because injuries happen no matter what, you know you need to work on your body, not your bike.
Just my opinion.
I know what you mean - I felt that way after the initial fit but it didn't involve cleats, so I think not having them perfect and doing a lot more riding than I had been has caused some bother. I also have a feeling that riding my other bike with a different Q (MTB) has exacerbated things. :(

abandon clipless for a few weeks and see how the knees fare. playing with position (moving them to extremes) isn't really sensible

I have some considerable knee pain too at the moment. Given the climate here, I wouldn't have expected to have any issues, but the humidity appears to be as bad the damp. Back to frozen peas and analgesics...

Pain peeps:
Do you spend 10 mins stretching pre-ride?
Do you ride fixed? Can you realistically gear down?
Have you had a proper bike fit and are you on the same setup they advised?
Have you seen a physio?If no to any of those, STOP WHINING (kindergarten cop reference).

An observation that may assist those who are afflicted with fixie knee.
The summer can be a happy collection of quick, "single" pints on the way home. As well as aggrevating any tendancy to gouty arthritis, dehydration can be an issue.
Try drinking a pint of water every morning first thing, then 11 am, lunch, 3pm and before bed, and see if this helps.

Post exercise stretching has proven benefits, stretching beforehand appears to be of questionable value.
And my knee pain required a menisectomy.
I ride 48/19 fixed, cyclefit and bike whisperer have done by bike fits. I have seen physios and do 5-8 hours of yoga a week.

Yes I stretch, even after my small 4 miles to work then the same again when I get home.
I've had a bike fit, which actually sorted my knee problems. I then decided to try out speed play and the pain came back. Saw another fitter to help and the pain went for a bit. Then saw a physio as I wanted shot of the pain, lowered the cycling at the same time pain subsided but was still there.
Since I've moved again I'm now cycling to and from work every day and my knee (just the one) is in a lot of pain. I'm fed up well and truly at my age I shouldn't be worried about my knees ffs.
Also I can't keep throwing money at something hoping it'll help. I'm considering seeing another fitter to help but really don't k ow what to do.

Decided to take this week off the bicycle - well until Thursday at least. I'll be icing and stretching everyday. Hopefully come Thursday I'll feel a little better. Then off the bike for 2 weeks after that as I'm on holiday, which would normally involve cycling but not this time :(.

Not sure if you said you had a pain free setup... If you did try going back to the setup that worked and don't change anything?

I have tried but the pain is still there. My fault for wanting to change pedals again. I wanted S.play but then wanted shoes I can walk in.
Talking about a pain in the arse. (Or knee)
Seriously getting annoying - think there's a place in my local shop that'll do it though.

I was led to believe that road cleats with float are generelly better for sensitive knees.

Yeah t'was what I was thinking. I like speed play but the maintenance thing is a pain. But suppose so long as my knees are better that doesn't matter that much.

Went to the physio at Kings College this morning (which was quite a challenge due to the flooding at Herne Hill). I have prepatellar and infrapatellar bursitis in my right knee. My hamstrings and calves are far too tight, I basically should be on a rack. Been instructed to stretch like I've never stretch before loads and go back at the end of the month. My left knee has been hurting whilst cycling which I think is connected to the lowering of my seat. This, coupled with the fact I have a leg length discrepancy, means I should have a bike fit according to evil physio.
