There have been a number of incidents around midwifery and an RCN survey in 2009 described in one unit as having a "gang culture" of bullying/hazing to see if you were tough enough, and don't get started on midwives vs junior doctors.
Add in decades old rivalries between CEOs of neighbouring Trusts, desperation all around to not be an outlier amongst your peers on any performance measure lest NHS England send in a team etc. etc.

The graphic might be incomplete, yeah. But there was this tidbit in the article:
Before June 2015, there were about two or three baby deaths a year on the neonatal unit at the Countess of Chester Hospital. But in the summer of 2015, something unusual was happening. infant mortality is around 3 per 1000, I'd imagine that NHS Trust would have around 3000 births per year, so you'd "expect" 8-10 per year.Certainly should have triggered investigations if they are that low then spiking to perhaps above national averages.

There’s always a non-zero possibility he could have made it to Argentina to live out his days quietly as an anonymous eccentric recluse, especially given his wealth and the experience of the people in his circle. It’s extremely likely he knew he was a marked man for making Putin/FSB look weak, but I’d wager it’s unlikely he’d head to a sympathetic country like Mali where a random wagnerite might recognise him 5 years from now. My guess is he’d see smoothing things out with Putin as his best shot not to die of nuclear dysentery, but there’s no bargaining with the devil.

Early reports are that he and his right hand man had just left a meeting with Russian army high brass, so it’s certain he would’ve been personally identified and FSB/spec ops watchers would’ve been trailing them overtly and also covertly to ensure he got on the plane, to the point they probably spoke with him on or near the tarmac.
Barring the novel shoot down a private plane element that TASS is officially reporting, it has all the hallmarks of a sanctioned FSB execution.
