The chain has completly bypassed the top jockey wheel in the mech, probably due to the mechanics 'skills'.

God thats only just in the dropout isn't it? Surely you can squeeze another link into that chain?

just to be the devil's advocate: the guy's running a flipflop hub with 2 gears, so maybe the one on the other side is 2 teeth smaller in which case the axle would be right at the end of the dropout and an extra link would make it impossible to tighten the chain.
