Sounds like you need a good quality spanner, get rid of that rack on the back of your bike. The bearings probably went because you didn't check the washers/lock nuts when you got them and thus got fucked bearings/cracked washers...
Oh and you didn't get a Dura-ace lockring! -

i use a craftsman forged open/closed 15mm spanner... so no problem there, the washers and lock nuts where fine when i built it the bike, and when i replaced the axel. and no i got a decent lockring when the stock one cracked... but no i just avoid all that mess, and ride better hubs, execpt on my beater.

I am currently rocking a formula on the back of my Eddy Merckx - it's a good street hub that stands up to bad weather and low maintenance and the track nuts grip better than the aluminium caps that came with my goldtec.

no i think they are rubish beacuse even the cheepo unseald suzie hub i had on my old fuji conversion out lasted them by 6 months.

but i have, and they are good hubs, but there aint a huge difference between them and formula from my experiance working on them both!!! i just dont like the stupid comment like get some "decent hubs" assuming the the poster has some amazing superiour knowledge. -

i sold my phils cheap because they wern't that different from other less expensive hubs.
believe me, as soon as i qualify and start seeing some money i will be all over some new wheels like stink on do-do.
just got the dura-ace lockring and even that is a 200% better than the tin that they came with...
