My DoE instructor swears never to wear earphones again.
He couldn't hear the lorry beeping him from behind to pull to the side.
Lorry went to overtake and killed a girl on another bike who the driver didn't see.

So because a lorry was trying to force someone to move out of the road another person died. That is NOT the fault of either cyclist. My girlfriend is profoundly deaf and cycles everyday on the road in London. She would most likely be able to hear a lorry horn with her hearing aids, but not different from someone with headphones. Not not mention that the wind stops you hearing as much as the headphones do.

Headphones always.
I don't think I have been on my bike in the last 20 years without headphones on (well perhaps a hand full of times).
I suppose it is what you are used to, but I feel in no way incapacitated by listening to music while cycling. You could even argue a case for listening to music while you are cycling being an encouragement to safe cycling, as you pretty much have to abandon sound as an aid to what s going on around you - and check everything out visually, you no longer pull out from behind some cars because you didn't hear anything, you habitually look, check everything visually.

Even drivers ask the deaf debate.. A lot of deaf people do have various hearing loss.

Try deafness that is slowly getting worse, tinnitus, baddd tinnitus on the right ear plus some warbled sounds that has stopped you wearing your right hearing aid since you were 13 :(

The built in speakers are generallu built for snowboarding aren't they? All the ones I've seen look pretty stupid if you're on a bike on the road, not to mention they must get pretty hot.

I tend to use these.
Quality is decent, not fantastic. But they don't really fall out, and you can still hear the ambience around you clearly.
Not pricey either.

I can't cycle wearing headphones, so in an attempt to bodge a version of Clefty's Creative Speaker Dock thing:
which I can't afford this month, I got bored at work yesterday and remembered I had some of these little speakers that came with a recent mobile phone.
I guess they're designed so you can sit on a bus and let everyone know what stunning taste in grime you have. But you can get 'em on the bay for £7 inc. postage.
Two cable ties later (ugly surgical ones until I dig out the normal black ones) and an ipod later, meant that El Guapo, Go! Ellis and I got to rock out to Earth Wind and Fire whilst cycling through Brixton last night.
Loud enough to be audible, and give me a tiny bit of tinnitus after some interesting song selections from the Ellis. Can still hear traffic noise over them, and it got pesky pedestrians to turn their head *the right way *before they crossed Atlantic Road this morning.

Great, I nearly ran over an old lady a few weeks back.
It was a red light, so she decided to walk in to the road about 5 meters before the crossing. Just because the cars are stationary doesn't mean there aren't anything else on the road.She had the cheek to turn around and give me an evil glare.
