Look at what happens to the steerer clamp bolt when you twist the extension, e.g. by pulling up on one side of the bar, as you do when sprinting.

Give DJ the points for getting the right answer. Placing the clamping bolt(s) at the back of the steerer means that the twisting of the extension, which tends to open up the clamp, gets translated into a nearly pure tensile load on the clamp bolt(s) by the camming action of the steerer in the split clamp. With the bolt at the side, twisting the extension will place a very large shearing load on the bolt in addition to the usual increase in tension.

Aesthetically it works - in a way that having the bolt at the back would not.
Is there an engineer approved way in which the aesthetic of the Feather stem could be kept?

Does it matter? The Feather is only ever going to be posed on, you don't think those mudguards will ever get grubby do you? Let alone an aesthetically unpleasing sprint with consequent loss of poise.......
