1 spacer is correct. More than 1 is anti.

A lot of pics, especially of new release bikes, come from bikeshop photos. They wont chop the steerer down too much as they have to sell the bike.

Very important on a carbon steerer (and alu I guess?), with carbon I'd rather the stem clamp it fully than have a little bit showing to allow the top cap to be adjustable.

Bollocks. Cannondale customer service said to me it wasn't needed at all. But leave it incase you might want to change position.
And I've smashed the hell out of my CAAD10 carbon steerer all winter, thanks to poor roads and it's been just dandy.
