I have a classic lugged bicycle that come with massive tyres and disc brakes.

Still make it [(40+6)x1.09]2.54 ~ 20 ins.
Where are you getting 1.09 from? That's usually saddle height from pedal centre as a function of inseam, so you need to add the crank length, then divide by 1.09
(40+6+16.5)/1.09=57.4cm or 22.6"
or
(40+6)/0.883=52cm or 20.5" This is the method suggested by Greg Lemond, but is particularly prone to error in small sizes as it fails to account for little people using disproportionately long cranks.

Anti. It would function the same upside down, while providing more tyre clearance, and it would function even better if the material were distributed evenly, instead of being pointlessly thickened in the middle. In fact, almost any shape and alignment would be better than the one they've chosen.
