16mm spacer for 1⅛" steerer is £7.50 posted to UK. Black anodised aluminium, 31.8mm outside diameter.

Rear derailleur problems: Trying to set up an XT M-8000 system, can only shift to the second lowest gear (picture attached below is in second lowest); after that, I have to push really hard to shift down further and the derailleur only moves ~half of what it should. Looking at it, seems that the cable is at an odd angle and can't really be pulled more towards the cable stop. Derailleur is mounted correctly, hanger is new.

Probably a fairly obvious solution to it, but I don't see it. Any ideas?

Your cable clamp washer should align with the shoulder on the body. I don't think that's the problem, but looking at it is hurting me too much to consider the real issue 😥

Have you checked that the low limit screw is backed out far enough?

Realized that myself, just quickly fixed the cable to take the picture, sorry for your eyes.

Limit screws are of course adjusted properly - thought this was the case at first as well, but the derailleur isn't even touching the limit screw when it seems like I'm almost breaking the shifter. Derailleur also isn't hitting the cassette or spokes or anything either.

Unhook the cable and see if you can get the upper pulley in line with the big sprocket by pushing the dérailleur manually. Once you know it will go to the right place, you just have to find out why the cable won't take it there.

I can do that (that's how I usually set the limit screws, is there a better option?). If you look at the cable on my picture vs yours, I think it's kinda obvious that, as the cage moves towards the lower gears, it reaches a certain point where the cable is bent in such an angle (like in my pic) that pulling on it further doesn't really move the mounting point any further outwards, but only tries pulling it upwards. Hope that's somewhat understandable.

Can you rotate the cable retention washer and retighten?

It does seem like a design which relies on the dropout not exceeding the specified thickness, as there's basically no margin for over-travel at the low gear end. I had the opposite issue with my MTB, dropout too thin meant it wouldn't reach top gear. Obviously, a shim solved that for me, but you're going to find it harder to get a negative shim.

That might actually be the issue, hanger seems a bit chunky. Tried it with a different RD and put in a new inner cable, didn't change anything. Will throw a hanger alignment tool at it tomorrow and if that doesn't solve it (more doing it just to be sure it's straight, not really expecting it to solve anything tbh), I'll try to get a friend to machine the hanger down a bit - I'm lacking around a mm of movement.

What bike is it on? If its a direct mount hanger you will have to remove the b link from the derailleur.

It's a normal hanger, not direct mount. But thanks for the suggestion, seems like I'm running out of somewhat obvious reasons why it shouldn't work.
