I've crashed like this before on a group ride with BCC
1 enter corner a bit hot but ok so hang onto the brakes into lean
2 surface is greasy so the rear whips out.
3 have to stop leaning not to lose the wheels but straighten up and run off the road
4 (now this normally means you end up on grass or something) but there was no run off so she hits the kerb and goes over the bars.It looks more dramatic than it actually is.

There may have been a bit of panic braking involved, but it looked like a classic Moto GP style highside to me. Once the back end gets sideways enough the back wheel grips suddenly and the rider gets pitched over the top.
I think the point about the ditches is more about the lack of run-off. Normally when a rider feels the wheel getting away from them they can countersteer and run onto the verge, but on that section there was no plan B on most of the corners, hence that really sickening impact.

That video must have been taken by the chap who ran up to her immediately after the crash.
I was angry at him for getting his phone out at first but to be honest, what the hell can you do as a spectator in that situation? I sure as hell would not be comfortable moving her at all. I've got quite a bit of experience in advanced first aid but I'm pretty sure all I could/would do is try to make sure other cyclists didn't crash into her and talk to her and try to keep her company while proper medics arrived.
So, someone with a better grasp of bike-physics than me explain what actually happened to VV? Back wheel starting sliding out, and then she went straight over the bars. Did she just panic and yank brakes? I know the side of the road was awful, but it didn't actually feature in her crash until she landed in it.
