The bar diameter is a sticking point I imagine. Perhaps you could fabricate a steel bar in a slimmer diameter to the standard 22.2 - and braze in a shim for the stem?
Titec produced a bar a few years ago called the 'hellcat' with a narrower diameter on the ends "for ladies" with tiny hands. (EDIT - 19mm OD) The bar was still a good width so unsuitable for your purposes but crucially: They also produced some correspondingly smaller grips which you could use if you made a skinner bar.

BMX stem is the way to go - And in terms of saddle / seatpost .. All the bmx guys ride those saddles which bolt straight onto the post now (through the top of the saddle) which gives a nice clean look (although most BMX posts are 25.4mm)

Odyssey Junior saddles are tiny and railed, they would be much more proportioned to the size of the bike. They are a bit dated now so you might have a bit of trouble sourcing one though.

Odyssey junior saddle ordered for a tenner, I will re-cover it with some snazzy suede.
Have offered child up to the bike, and it fits well, except for the saddle pushing her legs too wide apart, meaning she is on her tiptoes. The stem is actually a great length for her, so have ordered a nitto from the Foffa fire sale. I don't suppose anyone I know on here ever goes past their shop? I am having issues getting out of workshop/awayfromwife/child/dog.

Faster than me Ed. Better at spelling, and counting, and spacial awareness. And better looking. Better proportioned. Nicer hair. Much less rage. Better choice in friends. More productive. Other than that....
Mark 2 is in embryonic form (I have thought about drawing it). The question is whether her younger sister will get this one handed down, or a new one. My (second born) wife, is quite vociferous in her demands for equal treatment of them. I (a first born) am not really that bothered.
I will take some proper photos when I can get round to fixing my camera.
