Part failures does happen, but they're rare and the majority of failure I've seen in my shop were of installation error.

If you like you can examine it, the lockring threads are fine, it's just the ones that were on the hubs that have balded off.

Not tightening enough can mean when the cog slips it hits against the lockring with enough force to ride it off.
Had that been the case it probably would have happened far closer to the shop than the 3 miles away that it did.

the hubs is alu, the lockring/cog are steel, you can do the math.

Hah, it was something like -3 when I was riding home, went into the warm house, prop the bike next to the radiator, both tyres blew up.
It's more that I have a stupidly high pressure on the tyres 2 years ago due to listening to people ON HERE that higher = better (100 front, 120 rear).
Quite a few people did and it was because a batch or two of each were made with sub standard metals.
