Not sure about the 12 game ban but it is pretty nasty. Calvin is one of the biggest enigmas in football. Probably the best touch and holding up skills, awareness, flick on's etc of any player I've seen at bootham crescent. Looks absolute class but when it comes to scoring a tap in he's fucking useless. 12 years as a professional striker in the football league:

Not sure there's much in the way of mitigation there. The amount of time that run up took, if he'd spent it practising tap-ins he'd probably have scored 26 or 27 by now...
