back brake is much needed IMHO.
I've rode with trailer a lots, sometime pulling 120kg worth of stuff that required the used of disc brakes to actually slow down properly (pads got worn out after a couple months).
front and rear brakes make a massive difference, has not ridden it with a fixed wheel transmission though, but granny gearing is highly recommended, something like 50".
I've ridden it with 60" freewheel before getting an internal hubs, bloody hard work when there's a moderate load, especially up that hill in Wimbledon.
I found it quite interesting how an otherwise silent bike suddenly became alive with noises under the extra load. Made me think the drive train was going to disintegrate going uphill. I'm going to try it with a lighter gearing and a back brake first and then reinstate the gears if necessary. First job though, is picking my knees back up from the top of Crouch Hill where they left my body in protest.

I pull around 50kgs with my SS Swift around Putney on 51.6 GI. From the river up to the heath is manageable but I think I'd want gears if I was doing anything steeper than that tbh. Definitely wouldn't want to do it without (two) good brakes either.
