Looks like a repainted Gazelle Ventoux (lowish end bike). Or a low end factory replica with the same tubing.

Very classy this... the other frames are awesome but wheels, tyres, saddle bar tape combos are too matchy for my taste... have you been collecting long or are you an employee or ex-team rider?
