Hereford is in the west midlands. herefordshire is. so is warwickshire, worcestershire etc. think of the west midlands as being like london, and herefordshire being like some part of london. its pretty simple.

For the record, Wikipedia explains the difference quite clearly:

As a Warwickshire lad I try to distance myself from the grimy West Midlands as much as possible, especially as I am from Royal Leamington Spa.

I didn't like that map of n3lson's much as Leamington Spa was too prominent, so here's a better one which also shows the main motorways in the region. I'm not sure what that one round the outside is - maybe the M6 toll road?
