They addressed this momentarily quite well in Batman Begins. Tom Wilkinson tells him to quit his self righteous dogoodery as he knows nothing about real pain or struggle but then they just moved on so he could beat some people up. There's elements of empathy like when BM tells Harvey dent to think twice about beating up on the paranoid schizophrenic hood but he never seems to apply this logic to his own battles.
I agree though, the narrative is broke.
