When I am at the highest gear at RD (smallest sprocket) I can still shift using the smaller lever so the shifter may not be in the highest gear when I installed the cable ... but i swear I did it more than 10 times before installing cable argh
If there is no cable tension pulling the spool, just moving the lever when you're near the top gear position doesn't necessarily engage the next ratchet point. Once you have an inner cable in place, pull on it while hitting the upshift lever to make sure you really are in top gear position.
