Since it is used far more often as a term of abuse than as a vulgar slang term for a lady-garden, it is quite possible that there are people out there who know it means "an unpleasant or stupid person"(OED) without knowing that it also means "female genitalia, particularly the vulva"
With the -ard suffix, of course, it never refers to a body part, only to "a despicable man"(Macquarie)
