Like what? It wasn't put on right. Not much more info you can get on the actual wheel really.
The camera man suffered broken collar bone and cracked ribs but will make a full recovery.
The FIA have now ruled that everyone in the pit lane must wear helmets and cameramen can only shoot from the pit wall.
