Well considering you ride brakeless and if the cleat fails you're fucked, yes replace the damn thing!
Two it does look like the spring/bar has pressed into the sole, when clipped in can you feel any play?
Edit: riding with cleats too far gone could mean more play which would give the pedal a chance to push into your sole it seems.
