Stem failure is the stuff of nightmares but everything at fault in that review could be down to bad setup or it being badly built up. There isn't enough detail to discount that.
The issue with the mudguard clearance make you wonder whether he just needed another link in his chain, and therefore might not be the most sensible person. He is a bit crashy.
