I had the same issue as yourself recently, only slightly less severe. I've been riding with about 2 or 3mm of thread hanging out both sides. Hasn't come loose after 2 weeks of daily use
Come to think of it, every time I fit a shimano BB I have problems. I like every other shimano component I've used but their BBs are total shit. I think both the plastic cup and the shape of the splined tool/socket are massive design flaws. Won't be buying one again
