I ride in the primary position. If a car comes bombing down a side road and has a quick glance (thus not seeing me) and pull out, I'll still get t-boned.
Depends, lets say they are going to pull out across you as they wish to go in the opposite direction to you. If there is traffic coming in that direction then they are only likely to pull out half way if they have not seen you and there is no traffic coming from your direction. In this situation you could take to the centre of the road as you would when filtering.
If they actually wish to join the same direction of flow as you then again, moving out very wide to within a foot of the centre of the road will give you time to move that little extra if worst case happens.
