I could easily make an all over cover, then add a pen holder to that,
like this passport holder i made for someone, where only the two hard covers go into the pockets of the cover

Just made myself this, a little Credit card holder, with a fully sewn in Oyster card and a matching pocket on the back for notes.
Small enough to fit in a jersey or a smart suit pocket. (not that I wear either)
