If the old ones are loose it may be possible to pull them up again with a nut setting tool.
I have such a tool.
Alternatively you are probably best leaving the old ones in and getting new ones placed just above/below the original ones.

I really don't want to make any new ones, and I would have a go at re-tightening it myself, just there's a bolt already stuck in the fitting so I can't. I imagine that if I could cut off the outer, the inner will just fall out and leave a hole ready for a new nut.. no? Although i can't fit a blade in the tiny gap, and i don't want to bork the frame up any more..
Just thinking, if they made the profile of the inside part square, then you could shove a post down the tube, with a notch in it, and hold it in position from the inside.. unfortunately I can see down the seat-tube and its round.
