Before or after?
Of course,we all know that this is being done to cut people's benefit payments, and that it will be done on the cheap by the incompetent private provider with the biggest lobbying budget and the most former ministers/top civil servants on it's board, but it's also true that GPs have no incentive to maximise the productivity of their patients. Done right, a proper collaboration between health service and employer would be a win-win-win-win; lower benefit payments, lower healthcare costs, better health and higher productivity.
