I had the thread on a quill stem clamp sheer this morning. The stem was bought from them in May.
Overtightened? Perhaps, but only because the clamp wouldn't firmly hold the matching 3ttt bars without regularly slipping.
James at Planet x has had pics and is arranging a replacement. Which I think is very good of them, as it's not a clear case of either faulty product or misuse.
