This is such a persistent error that I've just spent 2 hours looking at it.
What I know is:
1) It's not a problem with the load balancer
2) It's not a problem with the shared file storage mounted to the backend servers
3) It works on some backend servers
4) All of the backend servers are identical (literally, as they are imaged from each other)
Which means:
5) I have no clue as to what the problem is
But as I think it's a file storage, permissioning problem more than anything... or something to do with NFS... I'm going to try something out to see if it helps.
Specifically I'm going to move attachment storage into the database, to see whether that solves it.
If it does, then this creates a different problem, but at least I know where to focus my efforts.
