Used a balloon whisk to get air into the milk whilst it was heating. Obviously, you won't get micro-foam, but it does froth quite nicely as long as you put some effort in.
These days, however, our kitchen features a Gaggia Classic (those wand attachments are rubbish!), so no more frother's elbow.
