I once hired a car to drive to Cambridge where I promptly opened the drivers door, causing a female cyclist to wobble and fall off her bike. I felt very bad and now always check and double check the mirror. You were distracted though.
On the bike I'm very wary of car doors, tend to ride further out in the road rather than be anywhere near them.
