were the police not called at the time? any accident involving injury has to be reported in at least a Collision Accident Report Book (CARB). Did he drive off? Contact the people who gave you their numbers as witnesses. Report it as soon as possible, if he reports it before you it will make it harder for you in the long run. Explain you haven't reported it earlier because you've been in hospital/recovering, in no way insinuate you haven't just got round to it (not that that is the case).
