You'll know all about changing hand positions regularly and shit. Another thing that helps is occasionally reorientating your head ie. twist head slightly to the left for a bit (people will think you've seen something interesting on the roadside) and vice versa. Even better, ride with your head tipped towards your left shoulder for a bit (and vice versa). You'll look like a dog who's just been asked a question, and possibly feel like one too.
And finally, actively pull your shoulders down and back from time to time.

I've managed to clear up bike-related neck pain by dropping my mileage to anywhere between 0 and 25 miles per week. Never felt better.
