MrsO would insist on them being changed. Ten years on she still asks whether I have checked my cleats almost every time I leave the house with a bike.

I've only had 1 broken clear, and it was a SPD-R.
Commuting I was bunny-hopping up a kerb, thankfully it snapped at the peak of the hop, and I landed cleanly enough on 1 foot.
Didn't realise it was broken (assumed it was a phantom unclipping) until it came my foot came flying out on the next sweeping left hander.Switched bank to MTB clips/ pedals as I'm much more comfortable with them.

Slightly off topic, but just dug a pair of Giro Empires out of the cupboard that I bought a few years ago because total bargain. In getting them set-up, I’ve found one of the cleat bolt holes has no thread in it at all! So can only put two screws in. Any idea if I could bodge this without dying, or should I just reserve these for turbo use?

See if you can get to them from the inside by taking the insole out

I believe giro sell spare threaded inserts, but if not I rode a pair with only two holes screwed in after I ruined the third hole trying to drill out a seized bolt and they were fine. YMMV etc

Try searching for t nuts. There’s the kind you put in ali extrusion which might work and the kind that dig themselves into wood which are what people have used to mount cleats to trainers etc I think.

Isn't it the norm for three bolt shoes to have fixed attachment points and its 2 bolt shoes where they are slotted?

No, at least not anymore for brands that care about bike fit. Maybe shoes at the cheaper end lack this adjustment, but giro empires are not budget 🤔
The new one is the premium cleat, the one attached to the shoe is the basic one - there’s less material missing than the comparison suggests.
