I quite like the aesthetic of that at the moment, it would be great to keep the outside exactly how it is but fully redo the interior (unless it's fine as it is - though I imagine there's a reason they've not shown it.)

If they own it, yes.
Maybe there’s some weird system where they pay for it but everyone else owns it. Mmmm.
I’d want to know exactly what the situation is because if you have no real access to it nor influence over how it’s used you might find maintaining the wall of house difficult and if you wanted external insulation (you do) you couldn’t fit it.

What's shown on the title plan/deeds, and is there any mention of a right of way over it? Might be worth purchasing the title plan/deeds for next door to check if there is any mention it there as well.
Hard to tell but it looks like both properties have gates to their rear gardens at the end of the passageway, so I would assume that the central of the fence runs through the middle of the passageway forming the boundary line.
Seems like the neighbour is pretty relaxed about bins being placed on their side but that could easily change should they sell etc.

Anyone had a structural engineer round to give specs for an RSJ for knocking a wall through? We've had one round, nice guy, he's quoted £200 for the drawings. Is that high or about right?

About right. I paid £155 (out of London) for advice and drawings in 2020 for 3 RSJs - one supporting wall, one chimney and bi-fold doors (which the builder worked off, bypassing an architect (which I'm not advising or otherwise btw). And my house hasn't fallen down yet.

I find this an odd one as I don't hate it aesthetically, it's just such an obvious "alternative" option.
But then if you don't want white what choices do you have?
