The only formal confirmation is that over 15% of Tory MPs have put a letter in.

I don't know how to find this out either but this (paywalled) article has some good background reading.
I can't find any record of somebody staying as leader for a full year after a VOC but that doesn't mean it hasn't happened ofc.

My guess is that the Govt had expected death figures from releasing hospital patients into care homes and there's evidence that they ignored it and did it anyway. Possibly even with a callous "it'll help sort out the pension crisis" too.
Just a guess though, we'll find out soon for sure.
