Is the top piece of the RD, the bit connected to the replaceable hanger, on those little bump stop grooves?
If you pull the RD up, does the B-screw always hit the "stop"? There's not a lot of meat to grip on there.
If there's no chain on, it moves up the whole block, right? ie. I assume you've checked the limit screws already.
