Now that the B-screw is all the way in to accommodate for the 40T the RD doesn't want to shift past halfway the cassette when the clutch is on. This is as expected I suppose to avoid too much tension/pull on the whole thing?
Or your chain is too short
Without the chain it moves 2 sprockets further up but not all the way to the end.
