The orange bit is the tooth and it's spring loaded against the disc. In normal operation it's in the green slot.
When you press the crash mode recovery button it spins the disc until the slot picks up the tooth.
The thing is changing some gears so it can't be in crash mode.
There's sometimes enough friction between the two parts for symptoms similar to what's described.
Anyway, we're mostly looking to properly rule it out before sending it back.
