Can’t see it in this eBay ad but my Sakae MTB stem has a small widened space in the stem slot for a cable outer stop, letting the bare cable drop through the centre of the stem body.

The Sakae has a low stack too! (okay, on the downside it weighs five tons).

When you start with such a fundamentally flawed design, the only way to make it work at all is to throw a lot of material at it.
