Some of the older diesel trains have windows on the door that you can slide all the way down. I've seen people hang their head out to have a fag.

A lot of the First Great Western have windows that you need to open to stick your arm out and open the door from the outside. One time I saw a couple of bemused tourists staring at the door wondering why it wasn't opening as the train left the stop they were planning to get off at.
