A title, description and the theme for the week e.g., "Architecture" and date of submission should be included in the entry.

The date is needless, since that is in the post metadata and is searchable.
The hashtag idea seems pretty good to me.

I disagree - I think it adds to post and it also means 'old weeks' are indexed without the need for pages of scrolling.
