Assuming the shim doesn't extend past the top of the stem, you'll be able to use a normal 1,1/8" top cap.
No idea about the best approach for the star nut, but @roboto has a CP thread where one method is discussed.
Cheers, thought maybe it'd have a little lip that sits in the internal diameter of the steerer so might need a smaller one.
No prob. The lip matches the stem internal diameter, so the steerer external diameter. As MDCC says you can chuck on an extra spacer. I've done this in the past but preferred to trim the shim so it didn't stick out.
