I think the first number (i.e. 45, 46, 47) is the outer diameter of the top of the head tube.
I think the second number (i.e. 2, 3) is the depth of the cap, so should be equal to (distance from top of headtube to top of compression ring, less a nominal tolerance to stop it bottoming out)

I need to get slammed on my Cannondale CaadX. Before I go measure the headset, just to make sure I have understood:
