That has freaked me out all day, but I've finally got to the bottom of it... he'd submitted the comment multiple times (possibly via mobile where the lag might defeat the anti-dupe code), and it resulted in 4 copies in the database at these times:
'2014-08-18 06:01:23.477836'
'2014-08-18 06:03:08.621653'
'2014-08-18 06:03:26.217209'
'2014-08-18 06:05:21.009638'Each one was counted as a new update, and thus appeared in your Following screen multiple times.
I'm going to suppress that when it happens.
