I'd drop

In my opinion, he does not deserve a licence to drive, he is unacceptably aggressive behind the wheel, and a danger when behind it. This does not bode well for his skills as a doctor and frankly is very upsetting.
It is particularly difficult for me as I am likely to come across him in a professional environment.

It is more likely to have a detrimental effect on your complaint than influence anyone in the way you wish it to. Stick to the facts.
