There's no point in sending it if you want to remain anonymous (which is completely understandable, but kind of invalidates it). Either try to talk to him personally (it shouldn't be too hard to find out who he is), making all these points in a calm, reasonable manner (may not be possible, I know, but despite being more junior, you're an assertive guy, and a doctor or NHS employee should surely appreciate that this sort of behaviour could lead to injury for less competent riders) or don't pursue it if you're worried about the consequences.
