If you can get wedge stems tight but not conical expansion ones like Cinelli, it would tend to indicate that the steerer is ovalised in the area where the wedge sits, probably caused by previous over-tightening of a wedge type stem.

Is there anything that can be done? Is it possible to shim it? It's pretty annoying!

When this goes back to track I might just stick a thread less adapter on anyway, so might not really matter.
