Thanks for the advice, Tester. My girlfriend has the bike at work so I can't investigate but I'm 99.999% certain that the steerer wasn't hitting the top cap.
It's been about a year since we built the bike and experienced this problem, at the time I managed to get just enough tension for the headset in the end. However, it's now loose and I just know any attempt to tighten it will likely result in the bike being unrideable. It's a daily commuter so I think I'll wait for the head doctor before faffing with it, just in case!
